Laparoscopic Splenectomy Using a Low-pressure Pneumoperitoneum for the Treatment of Immune Thrombocytopenic Purpura During Pregnancy: A Case Report

چکیده
We report the case of a 21-year-old pregnant woman who underwent a laparoscopic splenectomy for the treatment of immune thrombocytopenic purpura (ITP) because of severe thrombocytopenia refractory to medical therapy using steroids. The operation was performed during the 24 week of pregnancy using a rigid 10-mm scope with adjustable viewing angles and limiting the abdominal insufflation pressure to 8 mmHg.The operating time was 204 minutes, and the amount of intraoperative blood loss was 200 mL.Her postoperative course was uneventful, and her platelet count rapidly increased postoperatively. No additional treatments were required.Shinshu Med J 62 : 441―446, 2014 (Received for publication May 14,2014;accepted in revised form August 1,2014)

متن کاملTherapeutic Plasma Exchange in Pediatric Severe Immune Thrombocytopenia: A Case Report and Literature Review
Treatment of severe Immune Thrombocytopenic purpura (ITP) accompanied by life-threatening bleeding events is challenging and a combination of treatment measures should be undertaken to rapidly increase the platelet count. Herein, we report a case of severe ITP in a seven-year-old boy who suffered from massive bleeding which was totally unresponsive to conventional therapeutic interventions. Sin...
متن کاملAcute Ulcerative Colitis, Thrombocytopenia, and Venous Thromboembolism Treated With Combined Laparoscopic Splenectomy and Colectomy
Immune thrombocytopenic purpura and venous thromboembolism are rare but known complications of ulcerative colitis. Although several case studies have examined the treatment options available for ulcerative colitis patients presenting with immune thrombocytopenic purpura, there have been no reported cases that describe the optimal treatment for patients with ulcerative colitis and immune thrombo...
متن کاملA Case Report of Successful Management of Thrombotic Thrombocytopenic Purpura Syndrome (TTP) in a Pregnant Woman with Plasmapheresis
Background and Aim: Thrombotic thrombocytopenic purpura is a rare and life-threatening disease with a relatively high prevalence of 5% in pregnancy that causes thrombocytopenia. Due to the high prevalence and mortality of fetal and maternal mortality, which resolves rapidly with the onset of plasmapheresis, the diagnosis of this disease is clinically important. The aim of this study was to succ...
